Check all that apply for your camper.  If Not checked and initialed, we assume no permission and camper will Not participate in activity.  Person will sit on the side watching if short activity, stay at camp on field trip day and be assigned to another unit, or parent will be called to pick up child.  Mail vendor waivers (for kayakiing activities) to winona @ campsomd dot org.

*Transportation All buses need a parent to ride bus to/from camp.
All stops must have a bus stop monitor; all buses must have a chaperone. If either is not met, stop or bus could be cancelled.

Paddle Boating(grades K-12) If available.  I give permission for person herein described to participate in paddle boating at Gilbert Run Park. Personal Floatation Device (PFD) will be worn at all times. An adult or teen aide will be in the boat with younger girls.

Swimming - (grades 4-12) if available I give permission for person herein described to participate, traveling via school bus to pool.  Lifeguards may give water test for pool use at site and may or not be Girl Scouts. Girls go to Calvert Co. Prince Frederick indoor pool

Ice Skating - (grades 6-12), Teens only, If available.  I give permission for person herein described to participate in ice skating at the Capital Clubhouse in Waldorf.  Teens will travel via school bus on evening of June 25 and join day camp Bay Breeze girls.

Rock Climbing- (grades 6-12), Teens only, If available.  I give permission for person herein described to participate in the rock climbing wall at the Capital Clubhouse in Waldorf.  We will travel via school bus on evening of June 25. The time for this event will be after the ice skating is over.  If girls want to try this, bring $5 payable to Capital Clubhouse, and the signed waiver (link found on our camp website registration page).

Bowling - (grades 6-12 only) Teen Adventure units only, If available. I give permission for person herein described to travel to AMF Bowling Alley in Waldorf via car or school bus to bowl.

Late Afternoon, evening activity - Ice Skating [see above permission statements] (grades 6-12 only) - I give permission for person herein described to stay after camp on Monday evening and participate in special teen activities. School bus or carpool will be used to transport campers to activity.  Carpool pickup at end of event at the Capital Clubhouse. Time 7:00-7:30pm.

Ropes Course - (grades 6-12 only) Teen Adventure units only, If available. I give permission for person herein described to travel to Terrapin Adventures, a vendor high ropes course via school bus. This is a special all day Wednesday activity with extra fee charged at time of registration, if girls elect to do this.  A vendor high adventure waiver is required.  The link is found on our camp website registration page or here: waiver
